Facial Rejuvenation & Facelift, (Aging Face Management) Rejuvenation

What is Facial Rejuvenation? Rejuvenation Rejuvenation Rejuvenation
Rejuvenation
Our face is one of the most exposed areas of our entire body. Age, smoking, sun exposure, fluctuations in weight, genetics, exposure to pollution and other factors can all cause changes to our facial appearance. These changes are usually gradual, but one day, we might take a look in the mirror and find a different face than the one we saw years ago. Facial rejuvenation treatments offer a solution to those looking to regain a more youthful appearance.

Facial rejuvenation treatments come in many forms:

-Surgical procedures: such as facelifts, endoscopic face lift, contour threadlift, Blepharoplasty or Dermabrasion.
- Non-surgical procedures: such as Chemical peelsLASER resurfacing and Injectable fillers.

The main feature of Aging face is Wrinkles. So, What is the Wrinkles?

Four Types Of Wrinkles And How to Get Rid Of Them?!

face lines

Cosmetics companies don’t usually do a good job of explaining the problems they claim to solve. Take anti-wrinkle creams for example. Can someone just please tell me what causes wrinkles in the first place?

1. Atrophic Wrinkling Rhytides:
What they are: Fine lines on the face that are almost parallel to each other.
Where they occur: They show up in different areas of the face and body but they tend to disappear when skin is stretched transversally. (that means they shift when your body posture changes.) These wrinkles are associated with loss of elasticity.
What you can do: Since these wrinkles are due to underlying loss of collagen, you need to protect your skin using sun block agents. You can also use moisturizers to temporarily plump the skin and diminish the appearance of these fine lines.

2. Permanent Elastic Creases:
What they are: These are crease lines in the skin that become increasingly permanent over time, especially with sun exposure.
Where they occur: They show up most frequently on the cheek, the upper lip, and the base of the neck.
What you can do: Sun exposure makes this type of wrinkle worse. So unless you’re blessed with naturally dark skin, you should avoid sun exposure or use a sunscreen on these areas to prevent this kind of wrinkling.

3. Dynamic Expression Lines: dynamic dimple
What they are: Wrinkles that are caused by facial muscle movement.
Where they occur: Frown lines and crows-feet.
What you can do: These wrinkles respond to Botox treatments.

4. Gravitational Folds:
What they are: As the name implies, these lines are from the effects of gravity and they become increasingly obvious as skin begins to fold and sag. As we noted in a recent post, skin research on the International Space Station might shed some light on the mechanisms of gravity-induced wrinkles.
Where they occur: The location of these wrinkles is related to the thickness of skin. While we would have thought this means that thicker skin shows more folds, surprisingly the researchers said that a fat face may show fewer gravity folds than a lean face.
What you can do: Skin-lifting procedures are effective at removing these kinds of wrinkles.

Ideal forehead and eyebrows:

Forehead in Youth: Minimal laxity, No rhytids, Hairline irregular, Brow elevated and No fatty deposits
The aging face has common characteristics: Descent of the brow and mid face leading to hollowing of the around eye region. Lateral angle of the eye opening descent with attenuation leading to ectropion.

Eyebrow Position (Brennan’s “Ideal” Eyebrow)ideal eye brows
Women
- club shaped
- Medial end at level of base of the nose vertically
- Lateral end tapers and lies at extension of line which connect lateral angle of the eye (Lateral canthus) and base of the nose (alar base)
- both ends of the brow at the same level
- Apex (maximal height) on vertical line through outer edge of the colored part of the eye
- Arches above orbital bony rim
Men
- Curve shaped
- Thicker and uniform thickness than women brows
- Lies over supraorbital bony rim

Facial Rejuvenation Consultation:

During consultations, doctors can learn more about the patients and vice versa. This visit gives the doctor a chance to evaluate the current state of the patient’s skin and discuss options that will help the patient achieve a desired look.
Consultation visits also give patients the opportunity to address their goals and expectations for their treatment and to ask any questions they might have. These visits also allow the doctors to inform the patient of the benefits, precautions, treatment response, adverse reactions, method of administration and other important information.

I- Surgical procedures:

A- Facelift (Rhytidectomy)

What is Facelift (Rhytidectomy)?

There’s no escaping it, as we grow older, signs of aging begin to appear especially in the facial area. These include fine lines, wrinkles, deep creases, a jowly jaw line, and others. A facelift can offer a solution to these problems. However, a facelift cannot keep your face from aging but what a facelift can do is restore a more youthful appearance to your facial appearance. The procedure reduces the appearance of the most visible signs of aging.

During the facelift procedure, excess fat is removed and underlying muscles are tightened and the remaining facial skin is re-draped. This results in a firmer and fresher appearance. These results are dramatic or subtle, depending on the current facial appearance. Since each individual has his or her own unique facial features, the results vary with each individual. Facelifts are considered to be very individualized procedures.

The results of the facelift surgery can last anywhere from five to ten years, this also varies with each patient. As the years pass, your face will continue to age. Those who want to continue presenting a more youthful appearance can choose to repeat the procedure.

There is little debate that a facelift procedure is most effective in treating problem areas present in the lower facial regions such as the jowls and neck. More often than not, facelifts are commonly performed in tandem with other facial plastic surgery procedures such as upper and lower blepharoplasty which treat the facial region surrounding the eyes. Furthermore, in an effort to achieve a more profound rejuvenation of the face, injections of various collagen fillers or Botox may be utilized to treat pronounced creases in the forehead or Frown-lines between the eyebrow areas.

In an effort rejuvenate the appearance of their face, more and more people are turning to facelifts. The increasing frequency of facelift procedures places it among the top five most popular plastic surgery procedures including nose-reshaping (Rhinoplasty), Liposuction (Lipoplasty), Breast Augmentation (Augmentation Mammaplasty), and eyelid surgery (Blepharoplasty).

When considering a facelift it is important to recognize that, despite conventional opinion, the texture of the skin on the face will not be modified by the facelift procedure. Facial skin conditions such as fine lines, acne scars, age spots and smoker's creases will quickly return to their original texture and presentation regardless of how tightly the skin is pulled during a facelift.

Furthermore, a facelift will not result in bestowing beautiful skin on the recipient unless the patient possessed it prior to a facelift procedure. This being the case, there are solutions. For example, skin treatments such as laser resurfacing and chemical peels may be used in conjunction with a facelift in an effort to improve the texture of the skin and minimize the appearance of wrinkles, age spots.

Benefits of Facelift Surgery:

- While facelift surgery cannot stop the aging process, it may deter and offset some of its effects.
- Facelift surgery offers many patients a way to address the effects of heredity, sun exposure, environmental issues (i.e., pollution), smoking, etc.
- In some facelift surgery cases it is believed that the person receiving the facelift may appear to be a decade younger than their actual age.
Because of these various issues, facelift surgery continues to enjoy widespread and growing popularity.

Who would be best candidate for Facelift Surgery?

The best candidates for facelift surgery are typically those who:
- Possess good facial skin elasticity.
- Are in good physical condition
- Are psychologically stable
- Don't smoke
- Maintain realistic expectations
- Age: range varies significantly. For example, a person with fat deposits on the sides of their face or neck may be in their early twenties, while another person may be in their seventies when face surgery is most appropriate.

How is Facelift Surgery Performed?

- The Face could be divided into upper, middle and lower thirds. Each third has its lifting techniques.
- The traditional technique of facelift takes 2 to 3 hours, and done with general anesthesia in a hospital.
- The traditional technique for performing facelift surgery utilizes an incision that begins in the hairline near the temple and continues in front of the ear, around the ear lobe, behind the ear and back into the hairline.

- Following the precise creation of this surgical incision, the facial/neck muscles and sagging tissues are tightened, excess skin is removed and the remaining skin is repositioned in way that creates a rejuvenated and youthful appearance.

- In facelift cases where the neck needs specific lifting, a small incision is made under the chin from which the neck muscles and tissues are tightened.

- Following the lift process, we will use synthetic stitches or metal clips to close the surgical incision.

face lift procedure

N.B. Keep in mind that a facelift may improve the appearance of wrinkles but it will not remove them entirely from the facial region. Additional facial treatments such as chemical peeling and laser resurfacing, along with Botox © and filler injections may used to help treat laugh lines, wrinkles, furrows and other skin blemishes such as age spots and acne scars.

How much Pain is associated with Facelift Surgery?

While the degree of pain varies significantly from patient to patient, factors such as one's level of pain tolerance, the magnitude of the facelift procedure, etc., contribute significantly to the level of pain experienced following facelift surgery. Usually, there is very little pain associated with a facelift and residual swelling typically disappears within a few days. Additionally, most bruises are gone within a few weeks. Patients generally feel a sense of tightness and numbness that should gradually dissipate within several weeks.

Some patients are allowed to return home the same day as their facelift surgery, for some it may be necessary to spend the night in a hospital facility so that their recovery process may be monitored by trained medical staff.

What is The Long-Term Effect of Facelift Surgery?

Following the initial recovery period, patients may expect a rejuvenated and more youthful appearance.

While the immediate effects of a facelift may last for many years, the aging process continues. As a result, it is impossible to predict how long it might be before an individual would consider having a second facelift. Typically, those patients who look best after a facelift are those who looked best before the procedure. While some patients may have one facelift during the course of their life, other patients may have a second facelift performed seven to fifteen years later.

In regards to scarring, facelift surgery scars typically remain pink in color for the first few months before maturing into less visible white scars as time passes. Furthermore, facelift scars are primarily confined to the hairline region and within the natural creases of the skin.

What Are The Risks and Limitations of Facelift Surgery?

As in most surgical procedures, bleeding from cut tissues may lead to bruising and possible accumulations of blood under the skin resulting in topical discoloration of the skin and soreness.
Also related to most surgical procedures, any time the skin is cut infection is a possibility. As a result of new surgical techniques and drug therapies, infection is uncommon and treatable when and if it occurs.

How is Mini-Facelift (Limited Mid Facelift) Surgery Performed?types of facelift

- It is a technique for middle third rejuvenation. Mini-facelift takes 1 to 2 hours, and done with general anesthesia in a hospital.
- An incision that begins in front of the ear and around the ear lobe.
- Then Only excess skin is removed.

Endoscopic facelift (brow lift or Mid-Facelift)

A technique using an instrument known as an endoscope, a tiny fiberoptic lens inserted through half-inch incisions placed inconspicuously in your mouth and temple. The lens is attached to a camera connected to a television monitor, which allows the doctor to see where he is operating by watching the monitor and manipulating the tiny instruments externally. The endoscope also contains small tools.
During treatment, we can remove any fat bags near the eyes and jowls and treat lines and dynamic wrinkles but not excess skin.

This procedure benefits are: minimal wound and scarring, less bleeding and less possibility for swelling. However, it is time consuming (4 to 5 hours); it could not remove excess skin and should be done by well trained and certified consultant of plastic surgery.
